Here at Tiwari Watson Dental Care (TWDC), we would like to inform you of how and why we store your personal data as per the new GDPR which came into effect from 25th May 2018.
Information We Hold
The information we receive will include your name, DOB, medical/social history, address, phone numbers and email address. Referring dentists will also include the reason why you have been referred to our practice. This information is stored on a secure dental management system (R4 Carestream) which requires individual passwords to access. This system is backed up daily and a copy is stored overnight off site. We are committed to maintaining the trust and confidence of both our staff and patients. We care about the personal information we hold and how we share, alter or delete. We will ensure your data is only restricted to authorised personnel only.
Your information is held on our computer system until you request it to be archived or have ceased to be our patient for 10 years, this information is more difficult to retrieve.
Who We Share Your Information With
Greater Glasgow & Clyde Health Board, Practitioner Services, SCI Gateway (system via a hospital referral basis is carried out), Referrals to Orthodontic Practices, Specialist treatment requiring an external referral, Child Protection Services, Text Service Company (Texconnections), Newsletter Provider (Mail Chimp), IT company (IWT), Finance Company (V12 or Finance 4 Patients), Online Booking Service/Feed Back/Review (Patient Connections) Debt Collection Agency (P&J). If information is required by solicitors or a third party written consent will be required.
We use social media platforms Facebook, Twitter, Instagram and our own Website. Marketing Videos, pictures, reviews on these sites will be authorised by yourself prior to publishing. Under 16 years of age will not be used on any of our social media platforms, although their information maybe shared with some of the mentioned above bodies. Consent for children under the age of 16 must be signed by person holding “parental responsibility”.
Why We Hold Your Information
Your information is stored and maybe used to improve our care from your feedbacks, reviews and to improve our customer care and service.
To send your treatment details to NHS for payments and notes on treatment plans so we have detailed information on what procedures we have carried out and when.
Email addresses maybe used to send Newsletters, Referrals and to send you any information relating to your care ie: photos, consents, estimates, treatment videos/ leaflets requested by yourself.
Mobile numbers are used to send reminders regarding appointments and to contact you regarding certain aspects of your dental treatment.
Payments that are processed via card facility are kept securely and are then destroyed after a period of time has lapsed (Normally 6 months).
Change/Deletion of Details
If we need to delete any personal data we follow the procedure below:
- Paper copies of data (referral Letters, card payment slips etc)- Shredded
- Electronically – IT Company (IWT or R4 Carestream) to permanently deleted
- Archive in computer software system by practice
- Sharing of Files – NHS files are shared through Swan, encrypted interchange system
Your Rights
You can change your mind at any time if you decide to withdraw your consents.
You have the right to access your information that is held at the practice (this will be carried out within 1 month and is normally free of charge).
